Charitable bequests are a major source of income for charities but surprisingly little is known about them. The aim of this paper is to propose a multi-stage framework for analysing the bequest decision and to examine the evidence for Great Britain provided by new data on estates. The novelty of the framework is that it distinguishes between the different steps that lead to a charitable bequest. Our new data for Britain have the advantage of covering the whole population, in contrast to much of the US literature based on the small fraction of the population covered by estate tax returns. We focus on the relationship with wealth at death, on the form of the bequest, and on the different causes to which people bequeath.
Case report. We report a fatal case of a 37 year old gentleman who ingested a MCPA/bromoxynil co-formulation herbicide. Although clinically well on initial examination, our patient declined dramatically over his 18 h admission with increasing CO2 production, hyperthermia and metabolic derangement to eventually die from cardiac asystole 20 h post ingestion. Two hours after ingestion the MCPA concentration was 83.9 mu g/mL and bromoxynil concentration was 137 mu g/mL. Discussion. The patients' mechanism of death appeared to be uncoupling of oxidative phosphorylation, excess CO2 production and hyperthermia. There is limited knowledge on the acute toxicity of these herbicides, in particular bromoxynil, and this case highlights the relentless progression of severe toxicity in humans.

Sudden death is a major problem, with significant impact on public health. Many conditions predispose to sudden cardiac death and sudden cardiac arrest (SCA), foremost among them coronary artery disease, and an effective therapy exists in the form of the implantable cardioverter defibrillator. Risk stratification for SCA remains imperfect, especially for patients with nonischemic cardiomyopathy. Ongoing trials may make it easier to identify those at high risk, and potentially those at very low risk, in the future.
Objectives. This study was designed to verify initial observations of the clinical and prognostic features of hypertrophic cardiomyopathy caused by cardiac troponin T gene mutations. Background. The most common cause of sudden cardiac death in the young is hypertrophic cardiomyopathy, which is usually familial. Mutations causing familial hypertrophic cardiomyopathy have been identified in a number of contractile protein genes, raising the possibility of genetic screening for subjects at risk. A previous report suggested that mutations in the cardiac troponin T gene were notable because they were associated with a particularly poor prognosis but only mild hypertrophy. We consider a Markov chain in continuous time with one absorbing state and a finite set S of transient states. When S is irreducible the limiting distribution of the chain as t→∞, conditional on survival up to time t, is known to equal the (unique) quasi-stationary distribution of the chain. We address the problem of generalizing this result to a setting in which S may be reducible, and show that it remains valid if the eigenvalue with maximal real part of the generator of the (sub)Markov chain on S has geometric (but not, necessarily, algebraic) multiplicity one. The result is then applied to pure death processes and, more generally, to quasi-death processes. We also show that the result holds true even when the geometric multiplicity is larger than one, provided the irreducible subsets of S satisfy an accessibility constraint. ...
This dissertation is a transnational study that argues that a structure of mourning, spoken through and effected by the historical romance, underlies the narrative of national culture as it emerges in the Americas during the early nineteenth century. The writing, consumption and preservation of these texts reveal not only the psychic life of community but also the material basis for that psychic life. Writing and reading, the production and circulation of texts, plays a crucial role in developing this psychic life, and the historical romance was particularly important in the Americas for imagining a national legacy.
